NA, UPT & UT Testing

  All testing slots will be filled on a first come, first serve basis.  Test application and payment must be received in order to reserve a test slot.  In the case of a full test day, potential testers may be put on a waiting list. 

Test cancellation reimbursements will only occur if both of the following criteria have been met:
        1.  A replacement has been obtained for that cancelled test slot
        2.  The roster is full for that test day.

The amount to be reimbursed for approved cancellations is as follows:

       100% refund will be given if a cancellation occurs 60 days or more, before the test day. 

         50% refund will occur during a cancellation 30 to 60 days prior to test day. 

           0% or no reimbursement will occur for any cancellation less than 30 days before test day.

 
The chapter's Test Secretary reserves the full right to place a new tester, from the waiting list, in any of the approved cancellation scenarios mentioned above.  An individual that has cancelled and wishes to enter the tests again must coordinate with the Test Secretary.

All individuals confirmed eligible to test will need to fill out the "on-line" test entry application.  You will need Adobe's Acrobat Reader to utilize the on-line form.  Fill the form out via your web browser and keyboard, then print it out.  If you chose to fill in the test application by hand, then please print (...no cursive handwriting) and make all words as legible as possible.  Double check the form for accuracy and send it along with the payment to the Test Secretary - Steve Kane.  Make checks payable to "Appalachian Valley Chapter".
